Getting Started
The Initial Assessment:
The care process begins with an initial assessment. During this first meeting, I learn about your specific situation. This includes gathering the psychosocial and medical history of your loved one. After this assessment, we will develop a plan of care specific to you and your family.
Care and Support:
After reviewing the care plan, we begin providing care according to your needs. Depending on the situation, ongoing care can be as often as twice a week or as infrequent as once a month, and can involve a combination of home visits and phone calls. This care plan will help you prepare for lifestyle changes. It may include hiring help, knowing how and when to implement changes, and helping with the decision-making process. If your living situation is changing, I can help every step of the way and will make the arrangements during this time.
Monitoring and Follow-up:
Once you and your family are comfortable with the level of care provided and the situation stabilizes, our appointments will become less frequent. I will provide ongoing support and services as needed, and ensure arrangements continue according to your care plan. If the situation changes, we can revisit the care plan.
